State Department of Health denies license renewal application of former Clark County counselor due to felony convictions

The Washington state Department of Health has denied a license renewal application from a former Clark County agency affiliated counselor with felony convictions. 

Jacob Michael Green was convicted on two counts of first-degree criminal impersonation in December 2017 and was convicted on one count of conspiracy to violate the Uniform Controlled Substances Act in October 2018, according to documents released by the state Department of Health. 

Green obtained his agency affiliated counselor license in February 2023, with the state Department of Health requiring Green to stay on a two-year probationary period. 

He applied to renew his license in July 2024. The renewal request was denied due to convictions on his record, which the state Department of Health determined constituted as unprofessional conduct. 

The Department of Health’s decision was finalized on Nov. 11, 2024, according to the notice of decision.
